Medicare

Before we discuss specific Medicare plans, benefits, premiums, or costs available in your area, Medicare requires that I complete a few compliance steps with you.

You will be taken to a HIPPA COMPLIANT form that you need to fill in.  You must do this in one sitting (there is no save button) so please get yourself in a quiet space. You will need your Providers and your Prescriptions (if you want me to help you with those). I will be notified when you complete the form and then we can make an appointment happen!

I have many ways to meet:  in-person, video (Zoom) and phone.

Current Clients: If we're discussing your coverage for the current plan year and you've already completed this form, you typically do not need to complete it again unless your address, health care providers, or prescription medications have changed.

If we're discussing plans for the next plan year, Medicare requires a new form—even if you completed one earlier this year. A new Scope of Appointment is required for each plan year before we can discuss the details of next year's Medicare plans.
